Chronic condition care

Living with a chronic condition means a lot of check-ups, scripts and paperwork. We handle it in a way that keeps one doctor at the centre of your care, so you never have to start your story from scratch.

At a chronic care visit we check how your condition is doing — blood pressure, sugar readings, weight, side effects — adjust your medication if needed, and renew your repeat script. We'll also help with the forms your medical aid needs to register your condition on its chronic benefit, so your medication is paid from the right pocket.

Most chronic patients see us every six months, or more often while we're getting a new diagnosis under control. Between visits, your repeat script can be collected from reception without an appointment.

If your condition needs a specialist's input — a cardiologist, endocrinologist or pulmonologist — we refer you, stay in the loop, and carry on with your day-to-day care afterwards.

Good to know

  • Bring your home readings (blood pressure or glucose) if you keep them
  • Repeat scripts between visits can be arranged through reception — allow two working days
  • We help with medical aid chronic benefit registration
